Procedure to Obtain a Business Operations Permit

Do you want to open a business in Hazel Crest? The Village welcomes your entrepreneurship! Listed below are the procedures a prospective business owner must follow to obtain a Business Operations Permit in Hazel Crest.

1. Verify that the proposed business fits within the restrictions of the Village’s Zoning Ordinance for the proposed location.
 
2. If renovation of the site will be required prior to occupancy, you MUST:
  A. Determine whether a permit will be required for the work.
  B. If needed, complete the permit application, pay the permit fee, and supply drawings and/or specifications.
  C. If a sign is to be installed, obtain a sign permit by completing an application and provide a drawing of the sign.
  D. Arrange for periodic and final inspections.

 

3. Arrange for water service at the site through the Village of Hazel Crest Water Department, located at the Village Hall, 3000 W. 170th Place, Hazel Crest. The phone number is (708) 335-9603.
 
4. Applying for the Business Operations Permit
  A. Complete the attached form and submit floor plans/facility diagrams.
  B. Provide a copy of your Drivers License (we can copy)
  C. Pay the established annual permit fee
  D. Arrange for final, pre-opening inspections
  E. Opening can only be set when ALL code violations are taken care of.
  F. A Business Operations Permit and decal will be provided as soon as all requirements are met.

 

5. After opening, the Fire Department will conduct ongoing safety/property maintenance inspections on a schedule of frequency which is dependent on the type of hazard classification of the business.
